Does it include mounting hardware?
I can see how it connects the fork at the top but I may need clamps for the fork tubes.

It is designed to mount to a mid-fork eyelet and has mounting hardware for that, but not a clamp for the fork tube. The King Cage Universal Support Bolt might help you here. https://www.commutercycles.com.au/shop/king-cage-universal-support-bolt/
